|Transferring LIDDA Action
If an individual moves to a different LIDDA’s service area without changing program provider within a waiver contract area*, the transferring LIDDA must:

|After the transferring LIDDA has entered the data, the transferring LIDDA must send Form 8575 to the receiving LIDDA.
Within three days after the transferring LIDDA sends Form 8575 to the receiving LIDDA, the transferring SC must submit to the receiving SC a copy of the individual's:

Within five days after receiving Form 8575 from the transferring LIDDA, as described in Step 2 above, the receiving LIDDA must:
