TULIP Online Licensure Application System

Texas Health and Human Services Commission has introduced the Texas Unified Licensure Information Portal (TULIP), an online system for submitting long-term care licensure applications and health care regulation Architectural Review requests. TULIP replaced paper licensure applications for all provider types regulated by long-term care regulatory services, including:

  • Assisted living facilities
  • Day activity and health services facilities
  • Home and community support services agencies
  • Intermediate care facilities for individuals with an intellectual disability or related conditions
  • Nursing facilities
  • Prescribed pediatric extended care centers

TULIP is an electronic system that increases the efficiency of the licensure process by:

  • Making all licensure applications electronic, including those for initial licenses, renewals and changes of ownership
  • Accepting electronic payments
  • Providing immediate access to the status of licensure information

Important Information on Reporting of Resident Death

  • With the implementation of the Texas Unified Licensure Information Portal (TULIP) on Sept. 4, 2018, all reporting of resident death information must now be completed in TULIP. The deadline for reporting deaths is within ten workdays after the last day of the month in which a resident death occurred (Texas Administrative Code §19.606). This would make Sept. 17, 2018, as the deadline for reporting August deaths and Oct. 12, 2018 for September deaths. However, in order to mitigate the potential delay in completing registrations within TULIP, the deadlines for August 2018, September 2018, and October 2018, death reports are now due by Nov. 14, 2018.
  • In order to submit resident death information in TULIP, the Legal Entity that owns the Nursing Facility must first designate an individual as a security authority to register the Legal Entity in TULIP and provide access to the individual(s) who is responsible for submitting death reports.

What documents are required to submit an application?

Where and when do providers sign-in or register in TULIP?

Click on the TULIP Login button above or go to https://txhhs.force.com/TULIP to login. If it is a first time registration, please click on 'Not a Member'.

What should we do if we already are licensed with HHSC and have not received TULIP registration letter with the unique registration code?

Submit a help desk request using the web form found at: TULIP Support.

Which providers will use TULIP?

  • Assisted living facilities

What licensure activities will be completed in TULIP?

Providers will conduct all of their licensure activities in TULIP, including, but not limited to:

  • Submitting initial applications, renewal applications, and change-of ownership notices
  • Making electronic payments
  • Submitting resident death reports
  • Accessing the status of licensure applications and
  • Sending and receiving notifications and updates to HHSC related to the licensure process.

Until what time HHSC will accept paper applications?

A provider that mails a paper application to HHSC before Sept. 4, 2018, or a provider that receives a paper notice to renew its license before Sept. 4, 2018, will have a 100-day grace period during which HHSC will accept and process paper applications. However, providers who receive a paper notice may also renew their license in TULIP starting Sept. 4, 2018.

Effective Jan. 1, 2019, HHSC will not accept paper applications and will conduct licensure activities only through TULIP.

What type of credit cards will be accepted for payment?

All major cards including Visa, MasterCard, Discover and American Express will be accepted.

Will the providers be able to pay using the electronic check or ACH?

Providers will be able to select ACH payment type from the payment tab in TULIP.

Will providers be able to pay fill out application in TULIP and pay using physical check or money order?

Providers will be able to fill out application in TULIP and can print the payment coupon and send the physical check or money order to HHSC. Please note that the processing time will be longer if the physical check is mailed.

Who and when are the TULIP registration/security key codes sent to?

HHSC sent key code letters to all legal entities in August 2018. The key codes issued at that time have expired. Therefore, HHSC is sending each legal entity that did not use the first key code a new unique key code, via letter on June 28, 2019.

  • If the provider has already used the key code provided in 2018, they do not need a key code and will not receive a letter.
  • If the provider is a new provider since September 4, 2018, they do not need a key code for TULIP and will not receive a letter.

How will renewals be handled in TULIP moving forward?

Renewals applications will be auto-generated at the time of renewal and available under 'Pending Provider Action' section of the TULIP homepage.

How will the applicable fees be calculated in TULIP?

System will auto-calculate the fees depending on the selection made throughout the application, providers will be able to see final payment due on the payment tab.

How long does it take to receive the registration email after submitting the request?

Registration email from TULIP can take about 30-45 minutes.

Who is Security Authority and can the providers have multiple security authority assigned?

The security authority will be the individual(s) that will be responsible to approve or reject access request on behalf of your business. Each provider should identify at least one individual as a security authority.

Providers can assign multiple people as a security authority.

Will the providers receive renewal notice if they have not setup or completed TULIP registration?

Providers will not receive renewal notice if they have not completed initial registration. Providers must have logged into TULIP and entered their registration code to able to receive online renewal notice

Will all the registered individuals (including consultants) from the business be able to attest and submit the licensure application?

The security authority from the business should designate individual(s) responsible for submitting the licensure applications by clicking the checkbox while approving their access request.

Is Resident Death Reporting applicable to all the provider types?

Resident Death Reporting is only applicable to the death of a resident in a licensed nursing facility or licensed intermediate care facility for individuals with an intellectual disability or related conditions, or the death of a resident occurring within 24 hours after transfer to a hospital.

We forgot to enter registration code(s) during initial registration. How do we enter the codes after the account is created?

We are not able to submit applications and getting “you are not authorized to submit applications on behalf of this entity”?

How do we grant access to a consultant?

Can a user register multiple times using same email address in TULIP?

We highly recommend all the users to only register once to avoid creating duplicate accounts.

Is Social Security Number required for TULIP registration?

Social Security Number is required to create an account in TULIP.

Do all the new users have to complete the registration first before the security authority can grant them access to the facility/agency?

All the new users must complete initial registration once by clicking on ‘Not a Member’ from TULIP login page before security authority can grant them access.

To submit a Change of Ownership application in TULIP, what information does a buyer need to have from the seller?

Buyer only needs to have a Facility ID/License Number

What do we have to do after completing an online payment?

It is imperative that all the providers click on ‘Continue’ button at the bottom of the page after the payment is completed and submit the application. If this step is not followed, applications will not be considered as submitted.
