Purpose

Form SSA-2853 is a Social Security Administration form that provides an individual with proof that an application for a Social Security number (SSN) for a newborn occurred at the hospital through the enumeration at birth process.

Procedure

When to Prepare

When a person provides proof of an SSN application with Form SSA-2853 signed by a hospital official, the advisor accepts the form as verification.

Number of Copies

The hospital provides the newborn's parent with an original only.

Form Retention

Retain an imaged copy of the form in the electronic case record for the period of time specified in the Texas Works Manager's Guide.
