Board for Evaluation of Interpreters (BEI) certificates must be renewed annually. Certificate holders are required to complete this form and submit it with the applicable fee to renew certification.

Copies and Distribution

No copies are required unless a name change is requested. If a name change is requested, a copy of the marriage license or divorce decree must be included.


The program staff keeps the application one year after the applicant is no longer certified.

Detailed Instructions

This form is due on or before the expiration date printed on the certificate card. Certificate holders must send the completed form, along with the applicable fee to Texas Health and Human Services (HHS) DHHS at:

Note about Retention

Internal HHS users: In most cases, the original version of completed forms must be maintained in accordance with federal and state laws and HHS policy. If you are unsure how long to maintain a given form, consult the HHS Records Retention Schedule or Records Management Office.

Consumers: You may maintain a copy of the form for your own records if you choose.
