Purpose
Applicants who have received a passing score on the written Test of English Proficiency (TEP) and satisfy other eligibility requirements may apply for a performance test. This form must be completed and mailed with the applicable testing fee and other required documents at least 60 calendar days before a scheduled testing date.
Copies and Distribution
Enclose a copy of:
- a valid certificate card from the certifying agency,
- an official college transcript, and
- a valid driver’s license.
Retention
The program staff keeps the application one year after the applicant is no longer certified.
Detailed Instructions
Applicants must complete this form, enclose the required documents, and the applicable testing fee at least 60 calendar days before a scheduled testing date. Currently certified BEI interpreters are exempt from the education requirement. If the applicant has submitted an official college transcript previously, he or she should contact the Texas Health and Human Services (HHS) DHHS office to verify the transcript is on file. The applicant must check the appropriate box for desired test and indicate their preferred test site. The completed form, attachments, and fee must be mailed to:
