To file a complaint against an interpreter certified by Texas Health and Human Services (HHS) - Deaf and Hard of Hearing Services (DHHS), complete Form 3950: BEI General Complaints and submit it within 90 days of the incident.
You can also submit your complaint via letter, email, fax, or a digital device (USB stick or thumb drive, etc.).
