What We Do

Complaint and Incident Intake is part of Texas Health and Human Services. Their primary responsibility is to accept and triage complaints about health care providers regulated by Texas HHS.

  • Individualized Skills and Socialization
  • End Stage Renal Disease Facilities
  • General, Special and Psychiatric Hospitals
  • Home health
  • Intermediate care facilities (also called public and private group homes)
  • Hospice
  • Narcotic/Opioid Treatment Facilities

  • Outpatient Physical Therapy or Speech Pathology Service Facilities
