This report describes HHSC Value-based Care Strategy and Managed Care Value-Based Payment Programs, Quality Improvement Programs, Trends in Quality Measures, MCO Performance Indicator Dashboard, HIV Viral Suppression, Relocation to a Community-Based Setting and Statewide Initiatives to Improve Maternal Health Care. The report also includes information on the Quality Monitoring Early Warning System for Long-term Care Facilities

The Aging Texas Well Strategic Plan for 2024-2025 identifies the issues that older adults, informal caregivers, and aging services providers identified as top priorities impacting older adults; the strategies HHSC, other state agencies, service providers and other community partners developed to address these priorities; and progress towards implementing these strategies.

This report outlines how funding is appropriated to state agencies to meet specific social service needs for certain low income and at-risk populations through the Texas Title XX Social Services Block Grant. The Texas Health and Human Services Commission is designated to administer the Title XX Social Services Block Grant for Texas and is the lead grantee agency serving as the coordinator of funds appropriated from the federal government to various state agencies.

Per RP-49, each state agency shall develop a plan for conserving energy and shall set a percentage goal for reducing its usage of electricity, gasoline, and natural gas. 

This report provides information regarding the Foster Care Rate Modernization project, the proposed pro forma rate methodology based on DFPS’ final service descriptions, the methodological rates for the new system, and the fiscal impact based on DFPS’ implementation plan.

The Promoting Independence Plan is Texas' comprehensive response to the Supreme Court's 1999 Olmstead decision, but also outlines a vision and guiding principle for the Health and Human Services Commission to provide a system of holistic services and supports that foster independence and self-determination for people with disabilities so that they may be able to live fully integrated into their chosen communities. The goals of the 2022 PIP reflect several key areas essential to achieving this vision.

The report provides information about the experiences of CRCGs in their efforts to provide a coordinated approach to service delivery for children, youth, adults, and families with multi-agency needs. This report also includes recommendations for policy and statutory changes at agencies that are involved in the provision of systems of care services and recommendations for the statewide expansion of sites participating in the Texas System of Care.

Biennially, the Texas Health and Human Services Commission (HHSC) presents a report to the public about the provision of services at state supported living centers (SSLCs). Through this report, HHSC fulfills the mandate to develop a long-range plan containing information and recommendations regarding the most efficient long-term use and management of these facilities, operated by HHSC, as required in the Texas Health and Safety Code (HSC), Title 7, Section 533a.032(c), Long Range Planning.

The Housing Choice Plan is a stakeholder-led roadmap for a statewide approach for addressing gaps in housing and support services in Texas.
