Use
Certificate holders use this form to confirm that they have completed Continuing Education Units (CEU) necessary to recertify. The information on this form is also used as part of a criminal conviction record check from the Texas Department of Public Safety.
Copies and Distribution
Enclose copies of CEU documentation and/or official college transcript(s) to verify fulfillment of continuing education for the current five-year certification period.
Retention
The program staff keeps the application one year after the applicant is no longer certified.
Detailed Instructions
Certificate holders must submit Continuing Education Units (CEUs) at the end of a five-year certification period on or before the expiration date printed on the certificate card. Certificate holders complete this form and attach required CEU documentation, which may include copies of workshop certificates of attendance, Registry of Interpreters for the Deaf (RID) Certification Maintenance Program (CMP) documentation, or official college transcripts. The completed form must be mailed, along with documentation, and the applicable fee, to the following address:
