Purpose

This form is for Department of Family and Protective Services (DFPS) Child Protective Services (CPS) staff use. DFPS CPS must use this form to submit a risk evaluation for someone involved in a foster or adoptive home if that person:

  • is pursuing an association with a child care operation or home regulated by the Texas Health and Human Services Commission Child Care Licensing (CCL).

Procedure

To complete this form, fill in all fields with the appropriate information.

Once the Centralized Background Check Unit (CBCU) receives and processes all required documentation, CBCU will send you a letter notifying you whether or not the risk evaluation was approved.

Transmittal

CPS only submits the form to the Foster Adoptive Home Development (FAD) mailbox, along with the complete risk evaluation packet to FADCBCUBGChecks@hhs.texas.gov.
