Home Delivered Meals provides nutritious noontime meals delivered to people in their homes on a scheduled basis. Meals may also be delivered to a location other than an individual's home, provided the location is within the provider's service area and is approved by the case worker. Those eligible for HDM are aged 18 years or older. An interest list is maintained for people interested in applying for this program. This page contains information for HDM providers.

The Minimum Data Set (MDS) is a standardized collection of demographic and clinical information that describes a person's overall condition. All licensed nursing facilities in Texas are required to submit MDS assessments for all residents admitted into their facility. The current version is MDS 3.0.

As part of the Centers for Medicare & Medicaid Services’ (CMS) migration of the Minimum Data Set (MDS) submission and reporting functionality to their Internet Quality Improvement and Evaluation System (iQIES), CMS will turn off the current Assessment Submission and Processing (ASAP) system’s ability to accept any MDS transmissions on April 13, 2023, at 8 p.m. ET.
