Reporting Templates
QIPP SFY 2023 (Year 6) Documents:
- Component One PIP Reporting Template (MS Excel): Performance Improvement Plan (PIP): This document will be used to fulfill the condition of participation requirements. Facilities must conduct monthly activities and report progress updates twice a year, as published in the ‘Key Dates for SFY2023’ (PDF) document. (Updated 11/22/2022)
- Component Two PIP Reporting Template (MS Excel): Performance Improvement Plan (PIP): This document will be used to fulfill the condition of participation requirements for the third metric in Component Two. Facilities must conduct monthly activities and report progress updates twice a year, as published in the ‘Key Dates for SFY2023’ (PDF) document. (Updated 11/22/2022)
- Component Two: RN Staffing hours: Facilities must submit information through the QIPP Data Submission Portal as described in the ‘QIPP SFY 2023 (Year 6) Quality Metrics and Associated Performance Requirements.’
- Component Three: NFs do not have to report MDS data or results to HHSC for QIPP. NFs Report MDS Assessment Data to CMS, per federal requirements. HHSC will calculate NF performance each quarter based only on Medicaid managed care beneficiaries assessed during the reporting period. HHSC will provide NFs with additional details regarding Component Three methodology for calculating the Medicaid managed care beneficiaries when those details become available during SFY 2023.
- Component Four: Nursing Facilities must collect audit data for each month within the program year and report summaries to HHSC as published in the ‘Key Dates for SFY2023’ (PDF) document and ‘QIPP SFY 2023 (Year 6) Quality Metrics and Associated Performance Requirements’.
- Hand Hygiene Observational Audit Tracking Tool (Excel): This pre-programmed Excel workbook has been published by the Agency for Healthcare Research and Quality (AHRQ). It compiles hand hygiene observational data by individual, shift, position, location, and department to help staff regularly review opportunities for hand hygiene performance improvement. This tracking tool is used in conjunction with the Hand Hygiene Observational Audit Tracking Tool User Guide (PDF).
- Personal Protective Equipment (PPE) COVID-19 Observational Audit Tracking Tool (Excel): This pre-programmed Excel workbook has been published by the AHRQ. It compiles PPE audit data by individual, shift, position, location, and department, to help staff regularly review opportunities for improvement in donning and removing PPE. This tracking tool is best used in conjunction with Personal Protective Equipment (PPE) COVID-19 Observational Audit Tracking Tool User Guide (PDF).
- Antibiogram: Nursing Facilities are not required to use a specific template for reporting their antibiogram. AHRQ has offered a template.

Infection Control and Antibiotic Stewardship policies: Nursing Facilities are not required to use a specific template. Nursing facilities can utilize resources published by AHRQ, CDC and other agencies to tailor development and implementation of evidence-based policies and practices.

Rules
- 1 TAC Section 353.1301 generally applies to delivery system and provider payment initiatives.
- 1 TAC Section 353.1302 applies to the QIPP on or after September 1, 2019.
- 1 TAC Section 353.1304 specifies quality metrics for the QIPP on or after September 1, 2019.
