AP
alternate placement, any living setting outside the nursing facility
CCP
comprehensive care plan, an annual plan of treatment developed by nursing facility staff for individuals receiving care. The plan includes all needs: dietary, daily living skills, ambulation, level of functioning, etc.

customized manual wheelchair, a specialized service provided by the Texas Department of Aging and Disability Services

person in the nursing facility who works in the billing office and is responsible for completing and entering the Minimum Data Set and Preadmission Screening and Resident Review evaluations
