This program helps people 18 and older who are at high risk of becoming homeless find safe, affordable housing. It also helps them learn skills to keep housing and live independently. You must be eligible for services by the local mental health authority or local behavioral health authority.

Performance Contracts
The current housing measure uses information from the community data section, residence type field on the uniform assessment. Local mental health authorities are required to have at least 96.6 percent (Quarter 1 and 2 combined) of people served in a full level of care, and score a (1) or (2) in the residence type field to pass this measure.
