Updated DBMD Training Forms

HHSC has updated the following training forms used in the Deaf Blind with Multiple Disabilities (DBMD) waiver program to include the employment readiness service provider type:

• Form 6518, Deaf Blind with Multiple Disabilities Record of Completion for Person-Specific Training.
• Form 6519, Deaf Blind with Multiple Disabilities Record of Completion for General Orientation Training.
• Form 6520, Deaf Blind with Multiple Disabilities Record of Completion for the DBMD Program Service Provider Training.
