The Department of State Health Services (DSHS) wants to hear from you to learn more about the needs of current and former unpaid caregivers in Texas.
All responses are anonymous, and the survey should only take 15-20 minutes.
Who should participate?
Anyone who has:
- Provided unpaid care or assistance to someone with Alzheimer’s disease or other form of dementia in the past 12 months.
- Lived in Texas in the past 12 months, or the care recipient has.
Instructions:

- Please complete the survey by Monday, Sept. 30, 2024.
Please share the survey with other caregivers.
Please share this survey with family, friends, and/or community members who meet eligibility requirements as outlined above. Responses are needed from people of all backgrounds and in all locations across Texas.
