Renewal, Traditional Route
Traditional Renewal Route Process Map (PDF)
- Requirements: Nursing Facility Administrators (NFAs) are required to renew their license every two years. They must pay the $250 renewal fee and complete 40 hours of continuing education.
- Continuing education: Complete 40 hours of continuing education in the previous two-year licensure period.
- Includes one or more of the four domains of the National Association of Long-Term Care Administrator Boards (NAB).
- Includes the HHSC course in Infection Control and Personal Protective Equipment.
- Includes at least six hours of ethics training.
- All continuing education must be approved by the National Continuing Education Review Services, NAB, be an HHSC sponsored event, or be an upper-division semester credit course taken or taught at a post-secondary institution of higher education. For the upper-division semester credit course, one three semester hours course may be credited for up to 20 hours of continuing education.
- 20 hours of continuing education will be waived for approved preceptors who sponsored at least one Administrator in Training.
- Submit application through TULIP: Log in to TULIP to create an applicant registration, complete a Renewal application, and pay the $250 renewal fee.
- Military service members, veterans or military spouses who want to have the application fee and corresponding license fee waived, see the Fee Waiver for Military Service Personnel, Veterans and Military Spouse section of this page to learn more. The TULIP application cannot be submitted until the fee waiver is approved and the fees are marked as waived.
- Continuing Education tab: add continuing education hours, including course name, sponsor name, continuing education code (HHSC or NAB), date of completion, total CEU number, course number (NAB only) and number of ethics hours (if applicable).
- HHSC staff will review the application. If they find deficiencies, they will place the application in Response Required status so the applicant can make corrections.
- License issued: Once renewal application is complete and the licensure fee has been received, HHSC will renew the NFA license, and the applicant will receive an auto reply from TULIP.
- The licensed NFA will print their new license from TULIP.
Military Renewal Waiver Application
Military Renewal Waiver Route Process Map (PDF)
- Requirements: This application applies to military service members, military veterans or military spouse applicants who are licensed and hold an Active or Inactive NFA license in Texas. They must complete 40 hours of continuing education. These applications are prioritized.
- Continuing education: Complete 40 hours of continuing education in the previous two-year licensure period.
- Includes one or more of the four domains of NAB.
- Includes the HHSC course in Infection Control and Personal Protective Equipment.
- Includes at least six hours of ethics training.
- All continuing education must be approved by the National Continuing Education Review Services, NAB, be an HHSC sponsored event, or be an upper-division semester credit course taken or taught at a post-secondary institution of higher education. Inactive Status
Inactive Status Process Map (PDF)
- Requirements: A licensee has the option to place a license in a formal inactive status with HHSC. This allows the licensee to maintain an NFA license without completing CEUs. However, the licensee is not allowed to practice as an NFA until they meet renewal requirements.
- Submit application through TULIP: Log in to TULIP to create an applicant registration, complete an Inactive Status application, and pay the $250 inactive status fee.
- HHSC staff will review the application. If they find deficiencies, they will place the application in Response Required status so the applicant can make corrections.
- Inactive status issued: Once approved, HHSC will place the NFA license in Inactive status for two years from the NFA’s expiration date. An NFA may apply for inactive status for an additional renewal period (two total renewal periods) by following the same steps above. The NFA must renew a license in formal inactive status on or before the date that the inactive status expires by completing the Traditional Renewal steps. The Inactive NFA is not allowed to renew late.
Reinstatement
Reinstatement Process Map (PDF)
- Requirements: An applicant who previously was licensed and is in good standing in Texas may obtain a reinstatement of license without taking the examination if the applicant is licensed in good standing in another state, has practiced in that state for at least the preceding two years before the date of the current licensure application, and pays a reactivation fee.
- Submit application through TULIP: Log in to TULIP to create an applicant registration, complete a Reinstatement application and pay the $500 reinstatement fee.
- Documents tab: Upload license in another state (HHSC staff will verify license online)
- Work History tab: List employer name, city, employment start date, employment end date, employment status and job title.
- Complete FBI-based fingerprinting: Fingerprints for an FBI-based background check through the Department of Public Safety are required. Applicants will be provided with a link to complete the process.
- HHSC staff will review the application. If they find deficiencies, they will place the application in Response Required status so the applicant can make corrections.
- License issued: Once the application is complete and the reinstatement fee has been received, HHSC will issue the NFA license and the applicant will receive an auto reply from TULIP.
- The licensed NFA will print their license from TULIP.
Late Renewal Route
Late Renewal Route Process Map (PDF)
- Requirements:
- Scenario 1: A former NFA has up to one year after the expiration of a license to renew.
- Scenario 2: A military service member may request an additional two years (no extension) to complete a renewal.
- Scenario 3: A military service member, veteran, or military spouse may request a late renewal up to five years after expiration.
- Continuing education: Complete 40 hours of continuing education in the previous two-year licensure period.
- Includes one or more of the four domains of NAB.
- Includes the HHSC course in Infection Control and Personal Protective Equipment.
- Includes at least six hours of ethics training.

- Pay the license/late fee: $375 if 90 days or less past expiration date and $500 if 91 to 365 days past expiration date.
- Scenario 2: Submit a written request via email to NFA_Licensing_Program@hhs.texas.gov for additional time before the expiration date of the license.
- Provide documentation of status as a military service member (copy of current military service order).
- Scenario 3: Submit a written request via email to NFA_Licensing_Program@hhs.texas.gov for staff to allow access to a renewal application. Complete the Renewal application.
- Applicant must also complete the fee waiver process. See the Fee Waiver for Military Service Personnel, Veterans, and Military Spouse section of this page to learn more. The TULIP application cannot be submitted until the fee waiver is approved, and the fees are marked as waived.

- Documents tab: Upload documentation of status as a military service member, veteran, or military spouse:
- Military service member: a copy of a current military service order issued to the former NFA by the armed forces of the U.S., Texas or another state.
- Military veteran: a copy of a military service discharge order issued to the former NFA by the armed forces of the U.S., Texas or another state.
- Military spouse: a copy of a marriage certificate issued to the former NFA by a state of the U.S. or a foreign government, and a copy of a current military service order issued to the former NFA’s spouse by the armed forces of the U.S., Texas or another state.
- Complete FBI-based fingerprinting: Fingerprints for an FBI-based background check through the Department of Public Safety are required. Applicants will be provided with a link to complete the process.
- Applicant cannot be listed on the Texas Employee Misconduct Registry.
- HHSC staff will review the application or request. If they find deficiencies, they will place the application in Response Required status or will email the applicant for more information so the applicant can make corrections.
- Scenario 1: Complete a Renewal application.
- License issued or request approved: HHSC will renew the NFA license, and the applicant will receive an auto reply from TULIP. For scenario 2, the application will receive an email that the request has been approved.
- The licensed NFA will print their license from TULIP.
Name Change Application
- Requirements: An active NFA who needs to change their name. There is no cost for changing the NFA’s name.
- Submit application through TULIP: Log in to TULIP to create an applicant registration and complete a Name Change application.
- Documents tab: Upload a valid government issued ID and supporting documentation for name change such as a marriage license or divorce decree.
- HHSC staff will review the application. If they find deficiencies, they will place the application in Response Required status so the applicant can make corrections.
- Name updated and new license issued: HHSC will update name and the applicant will receive an auto reply from TULIP.
- The licensed NFA will print their new license from TULIP.
Updating Home Address
NFAs must update a change of home address within 30 days of change. There is not cost for updating the NFA’s home address.
- Log in to TULIP
- Select Personal Information tab
- Update to current address and save.
Fee Waivers for Military Service Personnel, Military Veterans, and Spouses
HHSC waives application fees if applicant submits the following to NFA_Licensing_Program@hhs.texas.gov:
- A written request for a waiver
- Documentation of military status of the military service member:
- A copy of a current military service order issued to the applicant by the armed forces of the U.S., Texas, or another state.
- Documentation of the type and dates of the service, training, and education the applicant received, and an explanation as to why the applicant's military service, training, or education substantially meets all requirements for licensure. This does not apply to an applicant for a provisional license.
- Documentation of status as a military veteran:
- A copy of a military service discharge order issued to the applicant by the armed forces of the U.S, Texas, or another state.
- Documentation of status as a military spouse:
- A copy of a marriage certificate issued to the applicant by a state of the U.S or a foreign government.
- A copy of a current military service order issued to the applicant’s spouse by the armed forces of the U.S, Texas, or another state.
- If HHSC requests additional documentation, the applicant must submit the requested documentation.
HHSC approves a request for a waiver of fees submitted in accordance with this subsection if HHSC determines that the applicant is a military service member, military veteran or a military spouse, and the applicant’s military service, training or education substantially meets all the requirements for licensure for a military service member. HHSC will mark application fees waived in TULIP. The TULIP application cannot be submitted until the fee waiver is approved and the fees are marked as waived.
